Summary:
As a Peer Support Specialist at Aspire Health Partners, utilizing lived experience to assist consumers engage in services, reduce harmful behaviors, and achieve and maintain recovery from substance use and/or co-occurring disorders.
Responsibilities:
- Responsible for interpreting and translating behavioral health communications, both verbal and written between consumer and clinician, ensuring accurate information is relayed.
- Providing individual and group sessions utilizing lived experience to help patients and clients with their day to day needs. Assisting case managers with MAI program goals and objectives
- Conducting and documenting HIV and Hepatitis C testing including pre-test counseling on all at risk clients and providing voluntary HIV antibody testing with the written informed consent of the clients accepting testing, providing post-test counseling to all clients accepting testing (Certification for HIV 500/501 within the first 180 days of employment)
- Documenting all sessions within Aspire EHR, using DAP note documentation style.
- Effectively presenting HIV and Hepatitis education and prevention strategies to all MAI clients
- Ability to transport and drive clients to and from important appointments, including clinical and doctors appointments
- Ability to work events and conduct groups outside of a normal 9-5 Monday to Friday schedule, working events on some Saturday and Sundays will be required as part of the peer support specialist role
- Identifies client need in a collaborative team based setting, by working closely with case managers, counselors, and program managers on a day - to - day basis
- Due to the population this grant serves, an individual with bi-lingual capabilities is preferred.
- A normal 40-hour work week may include weekend hours or later hours based on client availability and scheduling.
Job functions include:
- Providing direct support to program participants.
- Working with multi-disciplinary teams to gather information and provide the best wraparound care possible for clients.
- Documenting clearly and effectively in the Electronic Health Record.
- Following all program specific protocols in relation to specified role, maintaining fidelity to program model.
- Maintaining all required credentials necessary to perform job functions.
Qualifications
- High School Diploma
- Specialized certification as a Peer Recovery Specialist or Parent Coach may also be required, based on specific program
- Pass a Level II Background clearance
- Pass a Motor Vehicle Records clearance
