Intake Coordinator- Tampa, FL

Job Purpose: The Intake coordinator provides support to the Cohen Clinic as the first contact for services. This position will manage all intake calls and appointments, administer appropriate paperwork and psychometric screeners, and will schedule with appropriate clinical staff for biopsychosocial assessments. The position will conduct phone and face to face screenings and intakes on-site and off-site for veterans and their families. The Intake Coordinator will work with a multi-disciplinary team for case assignments, consultations and care coordination.

The Intake Coordinator must have strong organizational skills and good attention to detail, as well as good communication skills and an ability to work with multiple external partners. He/she must enjoy working within small, entrepreneurial environment that is mission-focused, results-driven and community oriented. Job Purpose: As a CCBHC Evaluator at Aspire Health Partners, your job purpose will be to perform the functions required to plan for, research and evaluate program services provided by Aspire Health Partners.

Job Functions

  • Assists in writing proposals and grants to secure current or additional funding to support the CCBHC model, with a focus on the evaluation and outcome sections of the funding proposals. Will actively participate in reports and renewal applications for grant-funded programming associated with the CCBHC model. When possible, identifies possible future funding sources and/or opportunities to help ensure continuity of funding for the CCBHC model.
  • Acquires and maintains knowledge of the requirements of the grants that are awarded and ensures that the programs are meeting these requirements. Periodically conducts reviews of all CCBHC related programming to guarantee compliance and fidelity to the CCBHC model. In doing so, will develop, implement, and oversee an organization wide evaluation procedure for evaluating the success of the organization in meeting the (9) core CCBHC service areas; and will collect, review, and analyze data from this evaluation procedure to compile into quarterly and annual reports to demonstrate organizational areas of strength and need, along with plans for enhancement of services. Reports should include specific data metrics to demonstrate progress towards goals (i.e. utility/productivity), objective timelines for enhancement/improvement plans, data related to consumer experience with services, service fidelity to evidence based models/CARF/statutory regulations, and employee training/education and experience; reports should highlight overall trends identified through the analysis of all data combined.
  • Based on research and sound practice, make recommendations for program structure and design, and will assist in strategic planning sessions to enhance the utility the program and the evaluation of it on an ongoing basis.

Job Functions

  • Oversee and facilitate the day-to-day operations and processes of Marchman Act and Baker Act involved cases across all AHP facilities, ensuring compliance with statutory and regulatory requirements.
  • Serve as a qualified professional for the Marchman Act, which includes, but is not limited to conducting involuntary assessments to assist in identifying the least restrictive treatment option based on ASAM criteria, ensuring the proper completion and submission of all court documentation, serving as the liaison with the court on Marchman Acts, completing court testimony, and ensuring all components of the Marchman Act are carried out in accordance with the law

What will you do? As a Care Manager at Aspire Health Partners, your job purpose is to respond, de-escalate, provide peer support, and directly connect individuals experiencing mental health related crisis in our community to the services they need.
